Help me out, is the appropriate greeting for addressing Japanese

1. Mr. Ichiro
2. Mr. Suzuki
3. Ichiro-san
4. Mr. Suzuki-san
5. Something else.
Is one more formal than the other?

先日会議で一緒になったTBと数通のメールのやりとりをした後、上記のようなメールが届いたので、下記のように返信した。

Dear TB,

Good question :-)

Mr. Ichiro, - Not common
Mr. Suzuki, - Formal
Mr. Suzuki-san, - Not so common (because "-san" means Mr. or Ms.)
Ichiro-san, - Casual
Suzuki-san, - very common in business scene
Suzuki-sama, - very formal
Ichiro-kun, - cacual & friendly
Ichiro - American style & friendly
Suzuki - very friendly but somewhat discourtesy

Please use Suzuki-san or Ichiro to call me.
